Event Request Form

Please submit the following form to request your event. Once your submission is received, the Special Events team will review your request and contact you within 3-5 business days to discuss approval and next steps.

Free or paid ticket event

Status message

You are required to submit a fully executed Event and Membership Deductibility and Insurance Worksheet, also known as the tax deductibility worksheet (TDW). The form must be submitted to UHF Special Events 6 weeks prior to event date.

Status message

For events requiring guest payment: EventBrite fees apply. All UH, UHF and UHAA events require the unit to absorb fees; guests do not pay beyond their ticket price.

Eventbrite Service Fee: 3.7% + $1.79 per sold ticket
Eventbrite Payment Processing Fee: 2.9% per order

For free events: no fees will apply.

Please submit your email request at least 10 business days prior to the requested send date of the email. The UH Foundation sends emails on behalf of all 10 UH campuses. All emails are scheduled in advance to avoid overlap with other scheduled emails.
